Essentially, it lets its eggs hatch within itself, and the growing maggot feeds off of "Milk" produced by its mother until it becomes big enough to crawl out on its on. Its first act is to pupate, and hatch into an adult fly.

It looks pretty uncomfortable, but at least the fly only has to do this once a week...
